Please print and fill out by hand any of these documents that are appropriate and send them to Michelle Larcomb before the beginning of the program. If you don't have access to a printer please send a message to Michelle Larcomb including your mailing address and you will be provided with a paper copy to fill out and return.

A copy of this file is needed for each high school you have attended. We will use this information to create a credit worksheet and a personalized plan to obtain your diploma.
